Public Health Solutions (PHS) and ModifyHealth are hosting a free community health and wellness event offering health screenings, backpacks and school supplies, children’s activities, food, raffle prizes, and on-site assistance with benefit programs for Medicaid members and their families. The event will connect families with practical back-to-school support and essential health and social services. The event will also introduce families to WholeYouNYC, a social care network connecting eligible Medicaid members across Brooklyn, Manhattan, and Queens with resources such as food assistance, rent and utility support, transportation, and more.
